About the position

TheSupport and Education Coordinator, First Link ,position works independently and as a part of a regional andprovincial team dedicated to the mission and vision of the Society.This position is responsible for consistent delivery of the Society’sFirst Link programs and services within an assigned region, includingreferral systems to connect clients to support services andinformation, and delivery of standardized workshops. Some travelwithin the region is required. A Hybrid work from home option isavailable 2-3 times a week.

This role reportsto Manager, Community Services Interior & North.

Key Responsibilities

  • Under the guidance of the Manager, Community Services,plan and implement support, education and information programs,ensuring that the expectations of service delivery are met and areconsistent with the Society’s strategic plan.
  • Coordinate thedelivery of standardized workshops for caregivers, people living withdementia and the public.
  • Coordinate the delivery of supportgroups for caregivers and people living with dementia.
  • Workwith the team to recruit and supervise volunteers for a variety ofroles within the service. Utilize Society accepted best practices totrain, evaluate and recognize support and education volunteers.
  • Coordinate and maintain the First Link referral process sothat people are connected to support services early in the disease andthroughout the journey.
  • Provide one-to-one telephone supportto people with dementia and caregivers, ensuring they are offered theSociety’s information, education and support services.
  • Develop and maintain relationships with health careprofessionals, allied health professionals and other stakeholdersthrough presentations, informal and formal contacts and informationexchanges to build, maintain and grow the referral network.
  • Maintain client records and statistical reports according toSociety and legislative policies, procedures and guidelines.
  • Other duties as required to meet the needs of the role inrelation to organizational goals.

About you

You have a post-secondaryeducation or relevant degree in health or social agency relatedfunction OR an equivalent combination of education and experience. Youbring related support and education experience in a human servicesenvironment which includes work such as public speaking, presentingstructured education programs, group facilitation, peer supportinterventions, volunteer management, and developing and buildingcommunication connections.

KeyQualifications

  • 3 to 5 years of experience in ahealth or social agency related function
  • Extensive knowledgeof and experience with Alzheimer’s disease/dementia and caregivingissues.
  • Must have knowledge of northern communities, and beable to support and participate in meaningful engagement of Indigenouspeople and their communities through culturally sensitive outreach andcollaboration.
  • Strong organizational and time managementskills, able to work independently, strong attention to detail.
  • Flexibility regarding scheduling is expected - some eveningand weekend work may be required.
  • Must have access toreliable transportation.
  • Travel within the region orprovince is required.

TechnicalQualifications

Working knowledge of MicrosoftOffice.
