Job summary

Winnifred Stewart in Edmonton is seeking a Support Coordinator to provide direct care and coordination for individuals with disabilities in their home and community.

You will develop person-centered plans, monitor health, document progress, and collaborate with families, staff and community partners. The role requires a Diploma or equivalent, strong communication, teamwork, and the ability to handle complex situations.

Qualifications

• Diploma or equivalent post-secondary education in a relevant discipline; with additional specialized training and progressively more responsible experience.
• A positive attitude and belief in the rights of persons with disabilities to define their life choices and participate in the community.
• Understanding of community resources and sensitivity to culture/ethnicity.
• Effective written and verbal communication skills.
• Well-developed coaching, mentoring and coordinating abilities.
• Strong time-management and priority-setting, balancing competing demands.
• Knowledge to model living, personal and social skills and to support person-centered plans.
• Minimum 2 years’ experience with diabetes, epilepsy, dementia, behaviors of concern, lifts/transfers, mental health disorders and dual diagnosis.

Responsibilities

• Help individuals develop, document person-centered plans guiding services related to development, community participation, behavior management and personal care.
• Assist with plans in complex situations and support health and well-being monitoring.
• Encourage rights-based choices and active participation in the community.
• Provide direct care for individuals with high needs as required.
• Maintain medical protocol adherence for medications and remedies.
• Develop recreational and social opportunities to expand supports within the community.
• Create clear records and complete required documentation; communicate professionally with families, staff and community members.
• Work effectively in a team-oriented environment and extend shifts when needed.
