Supervisor Population Health Clinical Services - Health Alliance

We are seeking a dedicated professional to support our team of telephonic Nurse Care Managers who conduct remote outreach calls. This role plays a key part in ensuring the quality and efficiency of care management services.

This position includes participation in a rotating weekend schedule.

Candidates must have a minimum of 2 years of supervisory experience.

A minimum of 1 year of experience as a Registered Nurse (RN).

RN licensure is strongly preferred.

Summary:

Responsible for supervising a multidisciplinary care team, overseeing daily care coordination services, participating in hiring and training staff, and conducting annual performance evaluations. Identifies and addresses barriers to care and streamlines workflows. Builds collaborative relationships to improve patient outcomes. Responsible for meeting productivity and quality goals.

Responsibilities:
  1. Maintain a safe environment for patients, staff, and others.
  2. Supervise and coordinate care management services, establish efficient workflows, and address barriers.
  3. Promote patient advocacy, shared decision-making, and decision aids.
  4. Use population data to identify at-risk patients, provide education, and remove barriers to care plan adherence. Monitor outcomes and collaborate to optimize care plans.
  5. Train staff on assessment, disease prevention, health maintenance, self-management, and navigating the healthcare system. Meet productivity and quality targets.
  6. Set standards for documentation, monitor compliance, and provide training.
  7. Collaborate with payers to address issues and prevent payment or service denials.
  8. Participate in meetings, problem-solving, goal setting, and performance improvement initiatives. Ensure policy and guideline compliance.
  9. Participate in annual performance evaluations to support staff development.
Other Information:

Education: Bachelor's degree required.

Licensure/Certification: Appropriate licensure for field of study.

Experience: At least 1 year of clinical experience in an acute or ambulatory setting caring for patients with complex conditions. Experience in team leadership, care management, utilization review, or quality management preferred.
