The Job Summary :
As a member of Community Support Services (CSS), a multidisciplinary team, the Overnight Stay and Dementia Services Supervisor plans, organizes, directs and supervises the activities of these programs to ensure effective operation and quality service delivery. In collaboration with the Manager and Director, the Supervisor is responsible for effective day to day operations of the unionized team ensuring monitoring of the required clinical programs, management of staffing, scheduling and medications.
Key Accountabilities & Success Criteria :

- Works in compliance with the Occupational Health and Safety Act and its regulations, reporting hazards, deficiencies, and contraventions of the Act, in a timely manner.
- Supervises and coordinates, in collaboration with the RPN, the completion of all components of medication management and reconciliation, monitors compliance with prescribed medications, and ensure care plans are completed within required timeframes.
- Provides continuous quality monitoring in the development and reporting of Quality Indicators.
- Coordinate the submission of data collection with the Manager and / or Director.
- Provide in the moment training and support to clinical staff.
- Assist with building and assigning CSS Learning Management System (LMS) modules and managing compliance for completion.
- Oversight for CSS staff orientation working in collaboration with CSS management team and Human Resources.
- Conduct audits, prepare reports and lead improvement activities based on the results.
- Assists the Manager, CSS, with the annual development of the Community Support Services (CSS) Quality Improvement Plan (QIP) and provides support with supervision and measurement activities throughout the year, as needed.
- Attends organization and departmental meetings and / or committees to provide input into both employee and participant care matters (i.e., Patient and Family Advisory Committee, Joint Health and Safety, Quality Committee, etc.).
- Supports the identification of efficiencies and the development of workflows within CSS program to enhance service delivery and staff effectiveness.
- Provides oversight and guidance on special projects within the department, ensuring alignment with program goals and timelines.
- Oversees the safe admission and discharge of participants in the CSS main site Overnight Stay program, by reviewing clinical risks, appropriateness, and case mix in collaboration with the care team.
- Promotes an environment that encourages and supports change using change leadership theory.
- Guides and supports staff in gathering relevant clinical information through interviews with participants, families, and caregivers.
- Facilitates, reviews and approves clinical assessment of participants’ mental health status; provides oversight in developing and adjusting care recommendations in consultation with physicians, family members, and the interdisciplinary team.
- Ensures documentation practices meet legal, professional, and organizational standards, including timely record keeping, secure storage, and proper authorization processes.
- Provides clinical direction to staff in supporting participant needs, including mobility, nutrition, and medication reminders, ensuring care is delivered with dignity and consistency.
- Oversee complex caseloads and provides guidance and support to clinical staff managing participant with multifaceted needs or behavioural challenges.
- Ensure participant and staff safety by identifying unsafe conditions or practices and take appropriate action to address them including reporting or removing hazards.
- Complete program evaluation of Overnight Stay on an annual basis and compile data to help guide quality improvement measures.
- Responsible for reviewing and resolving participant and staff incidents on RL solutions for the main site CSS programs.
- Collaborates with the Manager to support the development, implementation, and data collection of the annual Quality Improvement Plan (QIP).
- Support and implement the development of policies, processes and procedures that meet best practice guidelines in adherence with professional standards of practice.
- Participate in the accreditation process.
- Participate in community and organization committees, as requested, to ensure best practice.
- Provide ongoing consultation with team on admissions and suitability.
- Consults with Manager and Director related to decisions outside of established policies and procedures regarding suitability and eligibility of high-risk participants and access to crisis services.
- In accordance with CMHA privacy standards, the Supervisor communicates and exchanges health information with community and internal partners for the purpose of ensuring continuity of care (i.e. alternate LTC facilities), and contacts other divisional staff for services (food services, Business office).
- Leads and actively participates in supervision activities with management colleagues, including the development and review of policies and staff development initiatives.
- Ensures compliance with Ministry of Health standards, Collective Agreement, Accreditation requirements, applicable College standards and organization policies and procedures.
- Interviews, hires, and provides ongoing supervision and performance management of staff, ensuring role clarity, accountability, and alignment with organizational expectations.
- Prepares and maintains departmental manuals to ensure documentation is current, accessible, and reflective of best practices.
- Collaborates with the Director and Manager of Community Support Services to lead recruitment efforts, onboard new hires, and ensue appropriate staffing levels are maintained.
- Provides oversight to nursing and healthcare service delivery, ensuring the high standards of care are consistently met, and actively represents the program in internal and external professional practice forums.
- Identifies team training needs and leads design and implementation of development opportunities to support clinical competency and professional growth.
- Provides leadership in scheduling, shift change coverage, and staff redeployment across programs, ensuring safe, effective service delivery and proactive risk mitigation.
- Conducts performance appraisals for all direct reports, providing constructive feedback, coaching, and performance aligned with organizational goals.
- Promotes an environment that encourages and supports change using change leadership theory.
Qualifications :
- Registered Practical Nurse eligible to practice in Ontario and in good standing with the College of Nurses of Ontario.
- Bachelor’s degree in a health-related discipline, or equivalent, plus progressively responsible supervisory / leadership experience in a 24-hour direct service unionized environment, preferred.
- Minimum of three (3) to five (5) years experience in a health care setting. Community sector preferred.
Skills & Abilities :
- Knowledge and experience in CaseWorks EMR would be considered an asset.
- Knowledge of dementia and supporting persons with dementia, mental health, and addictions; long-term care and community program delivery; assessment and supportivecounseling; community resources; and pharmacology
- Ability to think critically in complex and stressful situations, and strong conflict resolution skills.
- Ability to foster a collaborative team approach for decision-making.
- Ability to use adult learning techniques in the delivery of formal learning / training sessions and presentation.
- Proficiency with various computer systems and software programs including database management, word processing, spreadsheet and presentation applications, (MS Office, CaseWorks etc.).
- Ability to independently, and as a member of a multi-disciplinary team, be adaptable and flexible in order to meet the demands of the ever-changing environment.
- Experience in conducting investigations and ability to conduct root cause analysis.
- Expertise supporting organizational change through strategic thinking and organizational behaviour / change management principles.
- Excellent communication, report-writing and presentation skills; critical thinking, problem-solving, relationship building skills and negotiation skills.
- Experience reviewing, handling and interpreting data would be an asset.
