Position Summary

Reporting to the Manager, of Integrated Clinical Projects and Administration, the Supervisor, Integrated Clinical Projects and Administration provides day-to-day leadership, supervision, and coordination of an assigned clinical support team working at the Mosaic PCN Clinic. They play a crucial role in ensuring the delivery of high-quality and consistent customer service and operational efficiency while fostering a positive team culture and leveraging team building opportunities to ensure a collaborative and cohesive team.

Leading a team of clinical support staff, responsibilities include setting expectations, developing staff through regular feedback, coaching and identification of development opportunities, as well as monitoring and evaluating performance, including conducting chart audits, data collection and review.

The Supervisor, Integrated Clinical Projects & Administration works closely with the Primary Care Managers to achieve programming goals; identify opportunities for program-and system-level improvements; drive quality improvement initiatives; and manage data collection and analysis to ensure continuous enhancement of patient care and clinic operations.

Key Responsibilities

People Management

  • Recruits, orientates, and supports the development of specific clinical support teams to ensure that Mosaic PCN clinic has consistent service and highly performing teams.
  • Provides support to the Manager, Clinical Quality in overseeing the day-to-day operations and performance of specific clinical support teams, including staff scheduling; team meeting preparation and delivery' performance monitoring and management; feedback, coaching and course correction when necessary.
  • Trains and mentors team members to ensure adherence to standards and guidelines.
  • Schedules regular one-on-one supervisory meetings with direct reports as well conduct probationary and trial reviews and performance evaluations.
  • Provides support and appropriate mentoring to teams to help staff overcome challenges, manage their workload, and deal with complex patient cases.
  • Coach team members towards improved competency and identify opportunities for professional development.
  • Acts as a role model for staff. Encourages teamwork, fosters a positive work environment, and promotes staff development.
  • Ensures that the appropriate controls, systems and procedures are developed and maintain to ensure accuracy, integrity and privacy of clinic and patient information. Ensures the adherence of best practice in clinic processes.
  • Monitors and evaluates the work of their direct reports, including performing chart audit, data collection and review.
  • Recognizes team members' contribution.

Program Planning, Evaluation, and Development

  • Assists in the development and setting of operational goals and key performance indicators in conjunction with the Manager.
  • Assists with the creation and implementation of plans to achieve operational goals.
  • Assists with the planning and development of clinic and team programming, patient interventions, and patient pathways.
  • Supports with data management including but not limited to monitoring charting accuracy and other performance metrics; data collection and reporting to assure quality service delivery; support quantitative and qualitative evaluation of processes and outcomes.
  • Escalates opportunities and risks through proper processes by leveraging the right stakeholders across the organization.
  • Enhances operational and clinical performance through data-driven collaboration with the QI team, using analytics and reporting tools to identify and act on improvement opportunities.
  • Supports the specific clinical support teams in adopting and implementing new processes and procedures.
  • Endorses team and organizational change as a champion for new initiatives.
  • Promotes the program and services internally and externally.

Communication

  • Demonstrated effective communication to ensure clarity, collaboration and alignment among teams, facilitating the seamless delivery of high-quality patient care and operational efficiency.

Relationship Management

  • Develops and maintains strong, collaborative working relationships with both internal and external stakeholders.
  • Works collaboratively with the Primary Care Managers to ensure all patients are provided with prompt, quality and consistent customer service, and all programs and teams are operating with high efficiency and quality.
  • Develops and maintains trusting and effective relationships with direct reports.
  • Acts as a role model and support for maintaining and strengthening working relationships within specific clinical support teams.

Customer Service

  • Ensures teams deliver the best possible customer service and care to Mosaic patients and staff, following best practices for trauma-informed and culturally sensitive interactions and care.

Quality Improvement

  • Supports the examination of workflow processes and implements changes as required for efficient clinic operations as per manager direction.
  • Participates in QI initiatives including Plan-Do-Study-Act cycles, literature reviews, process mapping, etc.
  • Ensures procedures and guidelines are accurate and current and participates in the regular review and updating of documentation.
  • Oversees the gathering and analysis of data related to programs within the portfolio.

Safety

  • Ensure OH&S and workplace safety practices are followed.
  • Reports incidents in a timely manner, taking action and ensuring recommendations.
  • Promotes a healthy work environment.

Performs other related duties as required.

Qualifications & Requirements

Education & Licenses

  • Bachelor's degree in healthcare administration or other health related fields is required. Consideration may be given to relevant combinations of education and experience.
  • Certification/training in project management is an asset.
  • Certification/training in change management is an asset.

Experience

  • Minimum of five (5) years previous experience working in a clinic setting.
  • Minimum of one (1) year experience in a formal supervisory or leadership role.
  • Experience with supervisory responsibilities in a unionized setting is preferred.
  • Experience working with and/or leading multidisciplinary teams.
  • Experience in program development, implementation, monitoring, and evaluation.
  • Experience working with diverse vulnerable populations. Experience working with refugees or newcomers is an asset.
  • Experience with quality improvement initiatives and methodologies is an asset.

Knowledge, Skills & Abilities

  • Excellent working knowledge of Internet Explorer, Word, Excel, Outlook and Teams.
  • Knowledge of Practice Solutions Suite an asset.
  • Knowledge of medical clinic office processes including workflow best practices.
  • Excellent time management skills with the ability to manage multiple tasks and priorities.
  • Excellent communication skills, using tact and diplomacy in all interactions.
