About the Opportunity
  • Early identificationof patients requiring complex discharge planning case management through high-risk screening criteria, bullet rounds, and referrals
  • Assessmentof patient’s medical, functional, cultural, and psychosocial needs, family issues/dynamics, and extent of community resources in light of input/assessments from the multidisciplinary pod and community partners re: identified needs and care options to facilitate the transition through care continuum
  • Planningwhich is a dynamic process and involves continuous review and communication of potential plans and timelines in partnership with the multidisciplinary team, client/SDM, family, and community agencies/facilities
  • Facilitationof safe patient discharge throughongoing assessment, case coordination, and development of appropriate discharge plans.Included but not limited topatient/family education; referral and completion of application to specialized treatment programs; coordination of family meetings; coordination of legal/financial capacity or SDM issues. Participation in bullet rounds, Joint Discharge Operations – JDO- meetings, and (where appropriate) weekly multidisciplinary team rounds. Ongoing review and monitoring of ALC designation; and ongoing re-evaluation of discharge plan especially if other special needs identified, e.g. significant behavioral/psychiatric concerns
  • Advocateto identify gaps in services, the need for individualized program consideration, and system-level changes required to meet the changing needs of the patient population

Activities:

  • Participate in unit-based rounds and conferences
  • Prioritize daily competing demands
  • Participate in weekly Joint Discharge Operations Meetings
  • Maintain an in-depth knowledge of community resources
  • Actively seek out new resources specific to patient/family needs and share these with other team members
  • Participate and contribute to departmental and program-specific meetings
About You

We would love to hear from you if you have the following:

  • Registered with the appropriate College or Professional Association
  • Baccalaureate Degree in Health Related Field - Nursing, Occupational Therapy, Physiotherapy, Social Work
  • Minimum of 3-5 years experience in Acute Care Setting
  • Previous management experience would be an asset
  • Demonstrated knowledge of services provided within Nova Scotia Health
  • Understanding of Utilization Process
  • Demonstrated ability to communicate effectively
  • Evidence of ongoing relevant education
  • Demonstrated good interpersonal skills and ability to maintain good relationships with patients, physicians, and hospital staff
  • Demonstrated physical ability to perform the duties of the position
  • Attendance records may be reviewed
  • Computer skills necessary to interpret data
  • Competencies in other languages an asset, French preferred

Hours
  • Long assignment Full-time position, 75 hours biweekly
  • Shifts include evenings,nights, weekends, and holidays
  • Travel to various sites may be required
Compensation and Benefits

$40.95 - $54.42 Hourly

Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
