Qualifications

  • Minimum undergraduate degree in human services or related field.
  • Registration with a professional college is an asset.
  • Minimum 3 years' experience in the substance use/concurrent disorder sector.
  • Knowledge of CBT, Relapse Prevention and Trauma-Informed Practice.
  • Excellent documentation according to standards.

Responsibilities

  • Individual counselling.
  • Case management.
  • Group facilitation as needed.
  • Teaching classes as needed.
  • Development of treatment plans based on client-centered recovery goals.
  • Assessment & referral.
  • Liaison with shelter case workers and attending case management meetings.
  • Collaborate with community partners to deliver wraparound support services.
  • Maintain electronic client files via EMHware according to agency standards.
  • Crisis/Conflict Intervention.
  • Discharge Planning.
  • After-care Follow-up.
  • Team consultation and decision-making.
  • Reporting on monthly service targets.
  • Participation in clinical supervision.
  • Other duties as assigned.
