Job summary

Oak Valley Health is seeking a registered Physiotherapist to join our interprofessional team. The role includes providing assessment, treatment, consultation, and education to patients and families across acute/subacute rehabilitation and community stroke services.

Responsibilities include supporting Community Stroke Rehabilitation Clinic patients, contributing to program development and quality improvement, and maintaining high standards in a fast-paced hospital environment.

Qualifications

• Current registration with the College of Physiotherapists of Ontario.
• Preferred 2 years experience in an acute care, subacute setting and/or outpatient care preferred.
• Previous experience with surgical, rehabilitation and stroke patient populations.
• Demonstrated knowledge of stroke rehabilitation best practices across the continuum of care.
• Ability to work effectively and efficiently in a fast paced environment.
• Demonstrated critical thinking skills in a subacute care environment.
• Excellent discharge planning skills with knowledge of community resources.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with the members of the interprofessional team, including support staff.
• Good computer skills and experience with electronic documentation preferred.
• Evidence of continuing education and ongoing professional development.
• Demonstrated good attendance and performance records with the ability to maintain these same standards.
• Demonstrates a commitment to a healthy and safe workplace for self and others by taking all reasonable precautions and working in compliance with hospital related policies.

Responsibilities

• Provide assessment, treatment, consultation, and education to patients, family members, and interprofessional team across acute care/subacute rehabilitation and community-based stroke rehabilitation services.
• Support patients within the Community Stroke Rehabilitation Clinic with a client-centred approach.
• Participate in ongoing program development and quality improvement activities.
• Adapt to frequent moderate and heavy exertion including lifting over 60 lbs and manual patient handling.
