Job summary

Qualifications

• Bachelor Degree Nursing (required).
• 3+ years nursing experience (required).
• 1+ year in case management, ambulatory care coordination, or related navigation experience (required).
• Master degree nursing (preferred).
• 5+ years nursing experience (preferred).

Responsibilities

• Assess needs for new patients referred to a specialist prior to their first visit.
• Document patient assessments and interventions in the EMR.
• Refer new patients to the specialty program’s supportive services.
• Provide educational and emotional support through intake and treatment planning.
• Coordinate scheduling with multidisciplinary providers and locations.
