Saint Elizabeth is seeking dynamic and passionate Speech Language Pathologists to join their Acute Care Transitions Program. The Acute Care Transitions Program ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, social support services, primary care, and acute care teams. Together, they create personalized plans that improve outcomes and ease the transition to home.
We are hiring dynamic, passionate Speech Language Pathologists (SLP) who want to be part of transformational change in home and community care by providing client-centered services. You will work in an environment that prioritizes team autonomy, offering high professional responsibility. You will be part of a diverse, collaborative care team providing services to community clients, conducting joint care planning focused on holistic, client-centered goals across physical, cognitive, social, emotional, and spiritual domains. Each client is assigned a primary care clinician who leads their care planning and facilitates interdisciplinary team efforts. As part of the SE Transitions team, you will be accountable for the care and outcomes of your clients.
