Summary Of Duties

To provide a full range of Social Work services to patients and their families of the Palliative Care Units K1E and K1C. This includes: supporting patients and families to meet their emotional and practical needs from admission to end of life or discharge, completion of psychosocial assessments, counselling, locating and arranging resources, consultation and collaboration with individuals and teams, project work, administration, and professional development. Candidates must have proficient skills with admission, discharge and transition planning.

Qualifications/Skills

1. Masters Degree in Social Work required
2. Member of the Ontario College of Social Workers and Social Service Workers in Good Standing required
3. Experience working in a health care setting (preferably 2 years)
4. Palliative end of life care experience required
5. Experience with the health care system, discharge planning and community resources, grief counselling and end of life care
6. Excellent assessment, interviewing and problem solving skills
7. Excellent written and oral communication, mediation, time management and organizational skills
8. Strong team building skills
9. Superior ability to manage multiple tasks and set priorities in a fast-paced environment
10. Familiarity with relevant legislation (HCCA, SDM Act, MAiD)
11. Strong negotiation and conflict resolution skills
12. Ability to be a team player as well as self-directed
13. Commitment to person centred care
14. Commitment to student education
