Location : Baawaating Family Health Team

Salary Range : $57,795 - $71,081

Who we are? We are a leader in First Nation Health. Our foundation is Indigenous Health in Indigenous Hands. We provide accessible, quality, culturally safe and holistic health care that supports and enhances the wellness of individuals, families, and communities. We work with our 11 First Nation community partners in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services and health promotion services. We have recently been approved as the first Indigenous led Ontario Health Team by the Ministry of Health.

The opportunity : Under the direction of the Program Manager of Baawaating Family Health Team (BFHT), the Social Worker will play an integral role in the care of the patients of the Baawaating Family Health Team. The Social Worker will assist the BFHT in achieving its vision and mission. The role of the Social Worker requires a thorough knowledge of mental health issues in a primary care context and the ability to function effectively in a team environment with physicians, other interdisciplinary health professionals and administrative staff. This role also requires a high level of interpersonal skills to deal with patients and their families in a holistic and compassionate manner. The scope of this service is within an inter-disciplinary team to provide primary health care to clients and their families within a client-centered care model emphasizing health promotion and client education.

What you need to bring with you :

  • Graduate degree in Social Work from a recognized university
  • Minimum of three (3) years combined education and experience in the human service field
  • Experience related to community based mental health and addictions preferred
  • Member in good standing with corresponding College and proof of professional liability insurance
  • Understanding of relevant provincial and local legislation and government policies, procedures, and practices pertaining to services provided by social workers
  • Knowledge of therapeutic models of individual, group, and family counselling
  • Expertise in brief treatment approaches and Cognitive Behavior Therapy an asset
  • Knowledge of client centered philosophy, basic principles and practices of community service delivery and short-term treatment options
  • Knowledge of community resources connected to the provision of mental health and addictions and related services
  • Demonstrated good interpersonal and networking skills
  • Ability to anticipate crisis and apply crisis theory
  • Demonstrated skills in assessment, program planning, implementation and evaluation, health promotion and community development
  • Knowledge of Indigenous and First Nation culture and traditional medicine
  • Flexibility and ability to adapt positively to change
  • Experience with EMR an asset
  • Ability to work flexible hours
  • Possess a valid Ontario Driver's License and a clear Driver's Abstract
  • Provide a clear criminal reference check
