Job summary

The Department of Supportive Care at the University Health Network is seeking a social worker to support outpatients with gynecological cancers at the Princess Margaret Cancer Centre. The role focuses on psychosocial care, assessment and coordinating with a multidisciplinary team.

Responsibilities include assessments, care planning, counselling, advance care planning, and grief support across the patient trajectory. A MSW, OCSWSSW registration, and 3 years of related experience are required.

Qualifications

  • Master’s Degree in Social Work (MSW) is required.
  • Membership in the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
  • Minimum 3 years related experience in outpatient and inpatient settings.
  • Excellent verbal and written communication skills.
  • Experience working with a multidisciplinary health care team.

Responsibilities

  • Conduct comprehensive psychosocial assessments to evaluate needs and service requirements.
  • Assess financial need for income supports and caregiver benefits.
  • Formulate and implement treatment plans with patients and families in collaboration with the multidisciplinary team.
  • Reassess and modify intervention plans as needed.
  • Liaise with community agencies and provide supportive counselling.
  • Engage in advance care planning and substitute decision maker documentation.
  • Connect families to grief and bereavement supports when appropriate.
  • Provide rotation coverage for solid tumor patients on inpatient units.

Position Summary

The Department of Supportive Care at the University Health Network was developed to provide care for the whole person at all stages of illness. Our team of experts work with patients and families to help improve their personal well-being and quality of life. This includes psychological treatment, supportive and palliative care, and rehabilitation services. We are currently seeking a social worker to join the Social Work team at the Princess Margaret Cancer Centre supporting outpatients with gynecological cancers.

Duties

The primary responsibilities of theSocial Worker include:

  • conducting comprehensive psychosocial assessments for the purpose of evaluating the nature of the problem and the need for service;
  • assessing financial need for relevant income supports and caregiver benefits;
  • formulating recommendations or treatment plans and implementing the intervention plan with patients and loved ones and the inter-disciplinary team for the purpose of facilitating comprehensive holistic care;
  • reassessing and modifying the intervention plan as required;
  • liaising with appropriate community agencies;
  • providing supportive counselling;
  • engaging in advance care planning including identification and documentation of substitute decision makers;
  • connecting families/loved ones to community-based grief and bereavement supports where appropriate;
  • Rotation coverage for solid tumor patients on the inpatient units.

This role requires performing activities required for the effective and efficient operation/maintenance of Social Work within the Department of Supportive Care and performing other duties consistent with the job classification, as required.

  • Completion of a Masters Degree in Social Work (M.S.W.)
  • Membership in the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Three (3) years related experience in outpatient and inpatient areas
  • Demonstrated mature interpersonal skills
  • Excellent verbal and written communication skills
  • Excellent problem solving, organizational and time management skills
  • Knowledge of policies, legislation, programs and issues related to social assistance institutions and services
  • Computer proficiency required (Microsoft Office Applications)
  • Experience working with a multi-disciplinary team in a health care setting required
  • Experience in an acute care setting with inpatients and outpatients required
  • Experience with serious illness including malignant and non-malignant illnesses preferred
  • Experience working with patients and families throughout the trajectory of care from diagnosis through to palliation an asset
  • Experience providing grief and bereavement support preferred
  • Experience teaching and/or contributing to curriculum design an asset
