Unison Health and Community Services is a non-profit, community-based organization that serves neighbourhoods in North West Toronto. Operating out of six locations, Unison offers core services that include primary health care, counselling, health promotion, Early Years programs, legal services, harm reduction programs, housing assistance, adult protective services, Pathways to Education and a Diabetes Education Program. Unison is seeking a Social Worker who can provide mental health support to clients participating in our Seniors Online program, an initiative designed to support vulnerable seniors in increasing their ability to virtually interact with service providers, family, friends, and online resources. This program improves access to information, promotes well-being, and reduces social isolation. The Social Worker will provide individual mental health assessments, short-term counseling, group facilitation, and case management support, while also building strong relationships with clients to meet their psychosocial needs. The Social Worker - Seniors Online will report to the Manager, Urban Health Programs.
Job Specific Responsibilities:
- Conduct individual psychosocial and mental health assessments for seniors participating in the Seniors Online Program.
- Establish trusting, supportive relationships with clients to facilitate their engagement in services.
- Identify mental health group learning needs and develop and deliver mental health education.
- Provide short-term counseling and case management support, including crisis intervention as required.
- Facilitate referrals to long-term mental health services, therapy, and other community supports as needed.
- Work collaboratively with the program team to evaluate and adapt the Seniors Online Program to meet the needs of participants effectively.
- Liaise with community agencies to address client needs and build partnerships to enhance service delivery.
- Maintain comprehensive client records, ensuring confidentiality in compliance with applicable legislation and professional standards.
- Act as an advocate for clients on individual and systemic issues.
- Work at various Unison locations to meet clients face-to-face, ensuring accessibility and flexibility in service delivery.
- Submit encounter data, administrative forms, and reports promptly to support program accountability and evaluation efforts.
Minimum Qualifications:
- Undergraduate degree in Social Work or related field
- Certification and membership in good standing with the Ontario College of Social Workers and Social Service Workers
- 3–5 years of working with seniors and/or complex needs populations, preferably in a community-based setting.
- Proficiency in current assessment and counseling techniques tailored to seniors.
- Demonstrated ability to develop and facilitate mental health education sessions.
- Familiarity with the social, economic, and cultural conditions of Northwest Toronto and the challenges facing seniors in accessing services.
- Strong interpersonal, organizational, and communication skills, including computer literacy.
- Bilingual or multilingual skills (English, Russian, Somali, Portuguese, or Spanish) are an asset.
- Flexibility to work at different locations and adapt to the needs of the program.
