Job summary

Major Responsibilities

As a Social Worker at Muskoka Algonquin Healthcare you will be working in the Seniors Assessment & Support Outreach Team (SASOT) department. Your responsibilities will include,

  • Assessment and coordination of supports and services for clients from the South Muskoka community (excluding those living in Long Term Care) aged 65 years or older
  • Client / family teaching and provision of strategies to maximize independence

As a Social Worker at Muskoka Algonquin Healthcare you will be working in the Seniors Assessment & Support Outreach Team (SASOT) department. Your responsibilities will include,

  • Assessment and coordination of supports and services for clients from the South Muskoka community (excluding those living in Long Term Care) aged 65 years or older
  • Client / family teaching and provision of strategies to maximize independence

Required Education & Certification(s)

  • Bachelor of Social Work from an accredited learning institution
  • Registration with the Ontario College of Social Workers and Social Service Workers in good standing

Required Skills & Experience(s)

  • 2 years of experience in the provision of evidence-informed practice in a community and / or hospital health care setting
  • Well-developed knowledge of the role of community agencies and services, and other professions in the care of patients aged 65 years and older
  • Knowledge and skills in geriatric assessment and screening
  • Experience in working within a multi-disciplinary team and interdisciplinary geriatric assessment

Desired Skills, Experience(s) & Education

  • MoCA (Montreal Cognitive Assessment) certificate
  • Familiarity with ADP (Assistive Devices Program) funding agencies
  • Familiarity with : depression in dementia scale, grief and loss training, government grants / pensions benefits, social housing
  • Staff referral program $1000 (For each successful permanent part-time and permanent full-time new hire you refer to MAHC)
  • Enrolment in the Healthcare of Ontario Pension Plan (HOOPP)

Additional Information

Location : The main site is South Muskoka Memorial Hospital (SMMH) located in Bracebridge, however you may on occasion be required to travel to Huntsville District Memorial Hospital (HDMH) located in Huntsville. Applicants must reside within 125km of one of our hospital campuses or be willing to relocate prior to starting.

  • Exceptions will be considered on a case-by-case basis

Hours of Work : Current shift is Monday-Friday 8 : 30 am - 4 : 30 pm; however, shifts and hours may change according to departmental requirements per the collective agreement.
