Overview

Safe Transitions is a non-profit organization providing life-changing support for women and children escaping gender-based violence. By offering affordable transitional housing and holistic services, we empower survivors to rebuild their lives and pursue a sustainable, positive future.

We are looking for a compassionate Social Worker to work directly with women who have experienced domestic abuse or human trafficking. You’ll play a key role in helping clients regain stability, access vital resources, and build the skills and confidence needed to transition into safe, independent living.

Responsibilities
  • Conduct intake assessments and develop personalized support plans
  • Provide ongoing, trauma-informed case management remotely via video or phone
  • Visit clients’ homes monthly to assess safety, provide support, and build trust
  • Monitor progress and adjust support plans to meet evolving needs
  • Connect clients to services including legal aid, financial literacy, mental health, and employment support
  • Collaborate with shelters, internal staff, landlords, and community partners
  • Support clients in securing long-term, stable housing
  • Maintain accurate, confidential case notes and participate in team case reviews
Qualifications
  • A valid G2 driver’s license (or higher)
  • Bachelor’s or Master’s degree in Social Work (BSW or MSW)
  • Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • 3+ years’ experience supporting survivors of domestic violence or vulnerable populations
  • Knowledge of Toronto’s affordable housing system and relevant community supports
  • Ability to work remotely with a reliable internet connection and private workspace
  • Strong communication, documentation, and organizational skills
  • Access to reliable transportation for monthly in-person visits
  • Demonstrated empathy, resilience, and commitment to empowering women from diverse backgrounds
  • Clear Criminal Record Check and Vulnerable Sector Screening required prior to employment
