Position Summary

Social Worker, Reactivation Care Centre Unit 2 & 3 Your goal in this position is to provide client-centered psychosocial assessments of the emotional, physical, cognitive, and social needs of referred patients and their families in the Reactivation Care Centre Unit 2 & 3 – A Central LHIN Hospital Collaborative. You will also assist the patient and family in developing and enhancing coping skills related to the social and emotional needs and problems that may accompany or predate illness / disability and hospitalization. You should have a strong network of connections in the care community to refer patients to community services, including retirement / long-term care, rehab facilities, and complex continuing care. Excellent interpersonal skills, collaboration, critical thinking, organizational, and conflict resolution skills are essential. The role supports the hospital’s academic mission through teaching, learning, research, and innovation. You will also ensure adherence to health and safety policies to maintain a healthy environment for patients, staff, physicians, volunteers, and families.

Responsibilities

  • Provide client-centered assessments of emotional, physical, cognitive, and social needs of patients and families.
  • Develop treatment plans in collaboration with the multidisciplinary team.
  • Formulate discharge plans with patients, families, and the healthcare team, utilizing community resources.
  • Coordinate and follow through on discharge plans.
  • Facilitate comprehensive discharge planning with the team and community resources.
  • Maintain knowledge of adjustment and grief counseling.
  • Maintain close liaison with hospital and community resources such as Community Care Access Centre, Social Assistance, Ontario Disability Program, and Drug Programs.
  • Advocate for patients and families to ensure rights and access to appropriate services.
  • Promote patient / family-centered care and interprofessional collaboration.
  • Provide consultative services to treatment team, hospital staff, and community partners.
  • Participate in MSW student supervision.

Qualifications

  • Master’s Degree in Social Work.
  • Registration with the Ontario College of Social Workers & Social Service Worker.
  • Minimum of 1 year experience in an acute care setting working with an interprofessional team.
  • Geriatric experience is preferred.
  • Excellent clinical skills and current professional standards.
  • Proficient with computers, MS Word, internet, data entry, and patient information systems.
