Job Description
In accordance with Northern Health's vision of an idealized system of services built on a foundation of Primary Care where people and their families receive primary care services in Primary Care Homes supported by interprofessional teams, the Primary Care Social Worker functions as a member of the interprofessional team. The role involves applying best practices to provide comprehensive assessment, care planning, and intervention for individuals and families. Working collaboratively with other healthcare providers, the Social Worker offers direct psychosocial care, counselling, group work, system navigation, advocacy, education, and linkage to relevant community resources, all while facilitating client self-management. The Social Worker is responsible for delivering social work services aimed at maintaining or improving the health and functioning of persons, families, and communities.
Additional Information
- Staff are expected to work in various settings.
- Shift Rotation / Hours of work: Monday to Friday, Days - 08:30 to 16:30.

Qualifications
- Bachelor's Degree in Social Work from an accredited university.
- Two years recent related experience in healthcare or related education, training, and experience.
- Current registration with the British Columbia College of Social Workers.
- Valid BC driver's license.
