Social Worker MSW, ABILITY Program (0.8 FTE, three-year contract)

Position Description

The Hospital for Sick Children is a leading tertiary pediatric center providing medical and mental health care to the most complex children and youth and their families. A dynamic clinical, education and research environment, SickKids is committed to excellence in integrated inter-professional care. It is a center of excellence in the delivery of innovative health care to the paediatric population.

Reporting to the Clinical Director, Social Work, this position will provide social work services to both patients and families within the ABILITY program (outpatient) at the Hospital for Sick Children. Through in-depth psychosocial assessments, the Social Worker identifies child and family strengths, problem-solving abilities, interactions, attitudes, feelings and cultural issues in relation to the child's illness. He/she also identifies supports and resources available to the family. This clinic supports children and youth with intellectual and developmental disabilities requiring tertiary level care at SickKids including severely restricted intake.

Here's What You'll Get To Do:

  • Conduct in-depth bio-psycho-social assessments and formulation focusing on children and youth with neurodiversity and their family caregivers
  • Develop and implement a plan of intervention which addresses coping, trauma, adaptation, treatment adherence and quality of life. This is achieved through a variety of modalities including counseling, psychoeducation, consultation, case management, crisis intervention, community outreach and advocacy
  • Support families in their adjustment to new diagnosis, as well as life changing acute and chronic illness
  • Support patient and families through short-term psychotherapy, using therapeutic approaches such as Cognitive Behaviour Therapy,Dialectical Behaviour Therapy, Emotion Focused Family Therapy
  • Support patients and families with coping difficulties, complex medical decision making, child welfare concerns, mental health concerns, and with securing resources related to the social determinants of health
  • Provide adjustment and supportive counselling
  • Commitment to trauma-informed and anti-oppressive practice
  • Collaborate with the interdisciplinary health care team and external services (ex. Attending school meetings or community team meetings)
  • Provide education to other members of the team in terms of social work interventions that benefit patients and families
  • Work collaboratively with a dynamic team in a fast paced ever changing environment (I.e. RN, MD, Case Manager, Behavioural Analyst, Dietician)
  • Integrate research into practice

Here's What You'll Need:

  • Master's degree in Social Work, preferably with Paediatric hospital experience
  • Registration with the Ontario College of Social Workers and Social Service Workers
  • Clinical experience with children and their families
  • Experience with child protection and/or trauma assessment and treatment
  • Well-developed family support skills
  • Knowledge of the network of community services and resources
  • Understanding of health care delivery systems and the practice of social work in a secondary setting
  • Ability to collaborate effectively as a member of the inter-professional health care team and to take initiative and leadership when appropriate
  • Excellent interpersonal, communication, organizational, time management skills and attention to detail
  • Demonstrated critical thinking and problem-solving skills
  • Demonstrated commitment to practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice
  • Proficiency in the use of electronic platforms such as Microsoft Office, hospital documentation systems and other relevant technology
  • To accommodate the needs of patients and families, working from home is not a possibility and the job is primarily on-site
  • Demonstrates commitment and actions in advancing equity, diversity and inclusion objectives
