Our interdisciplinary team comprising nurse practitioners, registered nurses, social workers, dietitians, chiropodists, and a pharmacist, works collaboratively with community partners to provide inclusive, culturally responsive health services. Position Overview
The Social Worker provides short-term psychotherapy, systems navigation, and group support to patients and families. Working within a primary care setting, the Social Worker plays a key role in program development, care transitions, and coordination with external service providers. This role reports to the Executive Director and follows the standards and ethics outlined by the Ontario College of Social Workers and Social Service Workers.
