Dignity Health Mercy San Juan Medical Center is a 384-bed not-for-profit Level 2 Trauma Center located in Carmichael, California. Responsibilities

The incumbent provides direct bereavement counseling, as well as social work services and counseling to patients and their families. In addition, provides expanded services in the areas of specialized bereavement (e.g. volunteer services, provides consultation with the Social Services team).

  • Meets discipline specific productivity standard.
  • Demonstrates competency in bereavement social work with hospice families during field observation by clinical supervisor.
  • Completes initial assessment of family bereavement needs and identifies risk factors as measured by designated evaluation tool.
  • Formulates, implements and documents the bereavement plan of care as measured by the designated evaluation tool.
  • Documents follow-up of identified problems.
  • Provides referrals to families to access bereavement community resources and individual counseling as needed.
  • Serves as a liaison for community bereavement support programs, We Honor Veterans program, annual memorial and other programs / committees as needed.
  • Facilitates community bereavement support groups or provides community education when requested.

Qualifications

Experience :

  • At least 1 year experience in hospice social work in the previous 3 years.

Education :

  • Master's Degree in Social Work from a school accredited by the Council of Social Work Education.

Licensure :

  • Current California driver's license, possession of automobile insurance, and access to an operating motor vehicle.
