Qualifications

Position Summary

Social Worker, Mental Health Inpatient: Your goal in this position is to provide patient and family-centred mental health care for patients in our Mental Health Inpatient unit across the lifespan. This includes conducting biopsychosocial assessments, developing care plans, liaising with community partners, and connecting patients to appropriate resources. You will assist patients and families in developing and enhancing social and emotional coping skills related to the needs and challenges that may accompany or predate illness/disability and hospitalization. You possess excellent interpersonal skills, and you effortlessly connect patients, families, and an interprofessional team. This position supports the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers, and families.

Key Responsibilities
  • Work in accordance with regulatory standards and professional practice guidelines, policies, and procedures for both North York General Hospital and the Ontario College of Social Workers and Social Service Workers.
  • Complete comprehensive patient-centred assessments of emotional, physical, cognitive, and social needs, risks, and strengths of patients and their families.
  • Prepare a treatment plan to address needs and challenges identified in the psychosocial assessment in collaboration with the multidisciplinary team and community partners.
  • Formulate discharge plans in collaboration with patients, families, and the healthcare team.
  • Guide patients in developing a crisis plan that supports their wellness and recovery after discharge.
  • Demonstrate effective collaboration as a member of the healthcare team and take initiative and leadership when appropriate.
  • Demonstrate knowledge and experience in individual and group therapy modalities.
  • Offer individual or family counselling where appropriate.
  • Maintain knowledge of hospital & community resources, e.g., HCC Coordinators, Income supports (OW, ODSP), Housing programs, etc.
  • Advocate for patients and families to ensure their rights are recognized and services are provided.
  • Use critical thinking, prioritize demands, and resolve conflicts effectively.
  • Provide consultative social work services to treatment team members, hospital staff, and the community.
  • Participate in MSW student supervision.
Qualifications
  • Must obtain and provide a current Police Vulnerable Sector Check.
  • Master’s Degree in Social Work.
  • Registration in good standing with the Ontario College of Social Workers & Social Service Workers (OCSWSSW).
  • Minimum of one-year experience as a staff Social Worker in an acute care setting.
  • Geriatric experience preferred.
  • Excellent clinical skills.
  • Proficient with computers, MS Word, internet searches, data entry, and patient information systems.
  • Bilingual in French/English is an asset.
