The Social Worker collaborates with healthcare team members to facilitate patient treatment and discharge planning. Responsibilities include assessment, care coordination, patient/caregiver education, and ensuring continuity of care for patients with complex needs. The role involves direct clinical services, psychosocial assessments, counseling, crisis intervention, discharge planning, resource referrals, and legal/guardianhip collaborations.

Requirements:

  • Graduation from an accredited graduate school of social work with an MSW degree
  • State licensure by the NJ State Board of Social Work Examiners
  • Licensed clinical social worker (LCSW)
  • Case Management certification preferred
  • One or more years of experience in an acute hospital setting preferred

Physical Demands: Extensive walking and talking; dealing with seriously ill patients and families; may assist in delivering DME to patient rooms.

Responsibilities include:

  1. Formulating psychosocial assessments and treatment plans
  2. Participating in case conferences and providing crisis interventions
  3. Providing counseling and discharge planning
  4. Assessing cultural, emotional, physical, and language barriers
  5. Documenting assessments and communicating with the multidisciplinary team
  6. Referring to community resources and legal agencies
  7. Assisting with patient transfers/discharges and legal issues
