As an integral member of an inter-professional team, the Social Worker (SW) for the Mental Health Services Team cares for individuals with mental illness. The Social Worker will provide support in the Outpatient Mental Health team, supporting individual psychotherapy, psychoeducation, system navigation, and group therapy. The Social Worker will support central intake and other clinics based on operational needs and for cross-coverage purposes.
In this role, the SW conducts intake assessments on clients referred to the Outpatient Mental Health Program to determine appropriateness of service. The Social Worker will also work closely with the interdisciplinary team to support patients seeking mental health support by conducting assessments, supporting stabilization, and providing follow-up. The Social Worker will collaborate with other clinicians, primary care providers, and community services to connect clients to appropriate resources and facilitate safe discharge. Additionally, the SW will contribute to program development initiatives within the Mental Health and Substance Use Program.
