Overview

As an integral member of an inter-professional team, the Social Worker (SW) for the Mental Health Services Team cares for individuals with mental illness. The Social Worker will provide support in the Outpatient Mental Health team, supporting individual psychotherapy, psychoeducation, system navigation, and group therapy. The Social Worker will support central intake and other clinics based on operational needs and for cross-coverage purposes.

In this role, the SW conducts intake assessments on clients referred to the Outpatient Mental Health Program to determine appropriateness of service. The Social Worker will also work closely with the interdisciplinary team to support patients seeking mental health support by conducting assessments, supporting stabilization, and providing follow-up. The Social Worker will collaborate with other clinicians, primary care providers, and community services to connect clients to appropriate resources and facilitate safe discharge. Additionally, the SW will contribute to program development initiatives within the Mental Health and Substance Use Program.

Education
  • Master of Social Work from an accredited school of social work, or equivalent degree, recognized by the Ontario College of Registered Social Workers and Social Service Workers (OCSWSSW).
  • Must be a member of the OCSWSSW in good standing.
  • Certificates in psychotherapy modalities (e.g., CBT, DBT, MI) are considered assets.
Qualifications
  • Minimum of 5 years experience in a Mental Health/Psychiatric/Substance Use services environment; knowledge and experience in mental health assessments and interventions required.
  • Experience with group facilitation in a hospital setting required.
  • Experience working with high-risk populations and crisis intervention skills required.
  • Knowledge of the Mental Health Act, Mental Health Reform, Privacy and Consent to Treat Act required.
  • Experience in community linkages and liaison development required.
  • Strong problem-solving, conflict resolution skills, and ability to contribute to high-performing teams.
  • Good work and attendance record required.
  • Adherence to legislated practices under the Occupational Health and Safety Act of Ontario is mandatory for all employees.
  • Commitment to maintaining a culture of safety and protecting psychological health and safety at work.
