Job summary

A Social Worker II is sought for the Claresholm Centre for Mental Health and Addictions, overseeing psychosocial assessments, discharge planning, and group work within an inter-disciplinary team. This full-time position requires strong communication, planning, and family-centered care skills.

You will contribute to individual, group, and family treatment plans, coordinate resources, and support patients and families through transitions in a rural Alberta setting.

Qualifications

• Completion of bachelor's degree in Social Work.
• Active or eligible for registration with the Alberta College of Social Workers (ACSW).
• Background in the mental health field with working knowledge of mental health disorders.
• Proven skills in communication, organization, and group therapy facilitation.
• Proficiency in Microsoft Office, internet, and email applications is essential.
• Experience with Case Management.

Responsibilities

• Provide a range of social work services to patients and their families, including psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, and case coordination.
• Coordinate discharge planning and transition planning in collaboration with the program team.
• Engage in administrative, educational, clinical and liaison activities within a multidisciplinary team.
