Job summary

Alberta Health Services is seeking a Social Worker II for the Home Living team in the Edmonton area. You will work within an inter-disciplinary team to deliver psychosocial assessments, counseling, and discharge planning for clients of all ages in home settings.

The role involves resource finding, crisis intervention, and coordinating care with internal and external resources, with competition for transition services and palliative care support as needed.

Qualifications

• Completion of bachelor's degree in Social Work.
• Active or eligible for registration with the ACSW.
• Basic Cardiac Life Support - Healthcare Provider (BCLS-HCP).
• Strong communication, time management and teamwork abilities.
• Proficiency with computer programs including Word, Excel, PowerPoint, Outlook.

Responsibilities

• Provide psychosocial assessment and resource-based interventions.
• Deliver individual, group and family treatment and counselling.
• Perform risk screening, case management and coordination.
• Plan transitions or discharges and engage in prevention activities.
• Collaborate with multidisciplinary healthcare team and families.
