Overview

Assisted Living Alberta is dedicated to enhancing care in the community, helping people be healthy, well and independent in their homes and communities. Edmonton Zone Continuing Care provides services through Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Social Worker II to join the Home Living team to provide professional support services in the homes of clients of all ages and stages of life. The Social Worker II works collaboratively as part of a multidisciplinary team and is involved in case management, including ongoing assessment, determining unmet needs, and care planning in consultation with clients to achieve client goals using internal and external resources. The role follows a Competency Framework that includes accountability, systems thinking, creative thinking, communication and relationship-building, and provides social work services to clients and families including psychosocial assessment, crisis intervention, education, advocacy, resource finding, consultation and referrals to the multidisciplinary team.

Responsibilities

As a Social Worker II, you will be a member of an interdisciplinary team and provide a range of social work services to patients and their families. Your services may include psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Qualifications
  • Completion of bachelor’s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Qualifications
  • Basic Cardiac Life Support- Healthcare Provider (BCLS-HCP).
  • Proven communication skills to work cooperatively and effectively with the healthcare team, patients and their families in a fast-paced environment.
  • Strong professional judgment, initiative, and time management skills.
  • Ability to demonstrate computer knowledge and proficiency (hands-on assessment may be required at interview).
  • Sufficient typing speed and ability to navigate Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.
  • Ability to work independently in clients’ homes, including various settings (home environments and weather conditions); ability to lift and carry medical bags weighing approximately 25–30 lbs.
Preferred Qualifications
  • Experience in a Continuing Care setting.
  • Knowledge of Connect Care.
  • Palliative Care experience.
