Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are seeking a Social Worker II to join our Home Living team to provide professional support services in clients' homes of all ages and stages of life.

Working collaboratively within a multidisciplinary team, the Social Worker II is involved in case management, including ongoing assessment, identifying unmet needs, and care planning in consultation with clients to achieve their goals using appropriate resources. The role requires adherence to a Competency Framework emphasizing accountability, systems thinking, creative thinking, communication, and relationship-building. Responsibilities include psychosocial assessment, crisis intervention, education, advocacy, resource finding, consultation, and referrals to team members.

As a Social Worker II, you will provide a range of social work services such as psychosocial assessment, resource-based interventions, individual, group, and family treatment, risk screening, counseling, case management, transition or discharge planning, and community awareness activities.

Required Qualifications: Completion of a bachelor's degree in Social Work and active or eligible registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications: Basic Cardiac Life Support - Healthcare Provider (BCLS-HCP). Proven communication skills for effective collaboration with healthcare team members, patients, and families in a fast-paced environment. Strong judgment, initiative, and time management skills. Ability to demonstrate computer proficiency, including Microsoft Office Suite, Visio, Outlook, and Internet navigation. Must be able to work independently in clients' homes, which may include various settings and weather conditions, and be capable of lifting and carrying approximately 25-30 lbs.

Preferred Qualifications: Experience in Continuing Care, knowledge of Connect Care, and Palliative Care experience.
