Position Summary:

The Geriatric Assessment Program (GAP) is a short-term specialized comprehensive geriatric assessment and consultation program for seniors age 65+. The interdisciplinary team of allied and medical geriatric specialists work together to optimize the health, function, safety, security, and daily functioning of seniors living in the community. Using the principles of client-centered care, the Social Worker will provide specialty services to clients and/or caregivers which includes in-home initial assessments and care planning, crisis support/counselling related to responsive behaviours, environmental interventions and crisis stabilizations, provide appropriate referrals to ongoing services (internal and external supports), monitor and evaluate the achievement of goals, provide information/education, consultation/collaboration, and advocacy. The Social Worker is also an integral part of the coordination and facilitation of the AGE-WISE Education and Support Program within GAP where he/she is responsible for small group education and support specific to the patient’s and caregiver’s identified needs. This position contributes to the hospital mission by being a team member, which ensures confidentiality, quality, and client-centered health care in the community. Responsible for participation in departmental, hospital and educational activities, which are supportive of professional and corporate objectives.This position will also help with potential coverage in all Outpatient & Inpatient General Rehab departments.

Qualifications:

Minimum 3 years direct experience working with seniors diagnosed with moderate stage dementia/neurocognitive disorder, responsive behaviours, and supporting their caregivers – Required

Proven crisis intervention skills and good knowledge of geriatric mental health illnesses and social problems - Required

Good knowledge of geriatric/dementia support services within Windsor and Essex County

Excellent interpersonal and communication skills with all patients, caregivers and community partners

Clinical skills to assess signs and symptoms of the “geriatric giants”

Experience with psychosocial and risk assessments

Good problem solving ability, and work well under pressure

Ability to work independently, and as a member of an interdisciplinary team

French Language Proficiency is considered an asset

Valid Ontario Driver’s License, means of transportation and ability to travel on a daily basis for outreach services - Required

Education and/or Certification:

Master of Social Work (MSW) - Required

Current Certification of Registration issued through the Ontario College of Social Workers & Social Service Workers – Required

PIECES Training- Preferred

GPA Training- Preferred
