Job Summary

We currently have exciting opportunities for Casual - Social Worker, Hospice Palliative Community Services at Langley Memorial Hospital in Langley, B.C. Responsibilities
  • Provide therapeutic and clinical counseling, including grief and bereavement services, to clients and families.
  • Conduct social and psychosocial assessments and interpret findings using professional standards.
  • Monitor and evaluate clients' responses to interventions and support care across hospital and community settings.
  • Collaborate with interdisciplinary teams to develop comprehensive care plans.
  • Inform and support clients and families about community resources, facilitating referrals and resource development.
  • Participate in quality improvement activities and contribute to program development.
  • Promote client autonomy and participation in care planning.
  • Develop and deliver educational programs related to psychosocial issues.
  • Maintain accurate records and reports according to standards.
  • Set professional goals and pursue ongoing development.
  • Perform other related duties as assigned.
Qualifications

Education and Experience: Master's degree in Social Work, two years' recent clinical experience with palliative clients, BC registration, BC Driver's license, and access to a vehicle.

Skills and Abilities: Knowledge of counseling and clinical practice, assessment skills, teamwork, community resource knowledge, effective communication, workload management, problem-solving, conflict resolution, and physical ability to perform duties.
