Hope to Health Clinic, Research and Innovation Centre in the Downtown Eastside is seeking a temporary full-time Social Worker to join our multidisciplinary team, providing therapeutic counselling, psychosocial assessments and care coordination.

Reporting to the Clinical Operations Manager and supported by the Professional Practice Leader, Social Work, you will deliver consultative and direct services, maintain records and coordinate community referrals within an interprofessional setting in

Qualifications

• Master’s Degree in Social Work from an accredited university.
• Two years recent related experience with marginalized populations in HIV/AIDS, substance use/addictions and/or mental health.
• Current full registration with the British Columbia College of Social Workers.

Responsibilities

• Provide therapeutic clinical counselling services and psychosocial assessments.
• Maintain patient records and coordinate referrals to community resources.
• Collaborate with interprofessional team to achieve patient goals.
