Job summary

The Hope to Health Clinic, part of Providence Healthcare, is seeking a Social Worker Lead to join the Downtown Eastside clinic. You will provide clinical guidance, mentor staff, and coordinate social work and psycho-social services within a multidisciplinary team.

Reporting to the Clinical Operations Manager, you will oversee practice standards, supervise multiple staff, and foster collaboration with community partners to improve patient care and outcomes.

Qualifications

• Master’s degree in Social Work from an accredited university.
• Four years’ recent health care experience providing services.
• Supervisory or leadership experience in a clinical setting.

Responsibilities

• Lead Social Work staff and supervise up to eight FTE staff.
• Provide clinical guidance and coordination of Social Work/Psycho-social and peer services.
• Perform direct patient assessments and treatment across substance use, mental health, and chronic disease.
• Coordinate with community partners to enhance services and promote quality improvement.
