Position Summary
Reporting to the Clinical Manager of Mental Health Outpatient and Social Work, the Social Worker (EPI Family Support) will provide outreach intensive services with screening, psychosocial assessment, therapeutic interventions, community and client education, accessing resources both clinical and community for clients experiencing psychosis and referred to the Prevention and Early Intervention Program for Psychosis (PEPP) program. Emphasis is on the early intervention of psychosis and on recovery and rehabilitation. This position is an outreach service of Mental Health Services at Brantford General Hospital.
Primary Responsibilities
- Provide intensive social work services to Families and Caregivers referred to EPI and Mental Health Services and be a team member of both interdisciplinary Adult/Child Mental Health teams.
- Provide service to clients utilizing acceptable methods of delivery of social work service. This requires competence in patient and a family education, the assessment and screening of individuals, couples and families and abilities to utilize the treatment modalities of intensive case management, individual therapy, marital/family therapy, and group therapy.
- Awareness of and ability to use/collaborate with community resources to assist clients to maximize their recovery.
- Work in collaboration with district supports in addressing specialized treatment approaches for individuals experiencing a first episode psychosis.
- Deliver services in community settings as well as offices in Brantford, Brant, Haldimand, Norfolk and clients' homes.
- Work within EPI philosophy in supporting individual clients in their recovery by assessment of their needs and development of goals. (E.g. leisure/recreation, housing, medical, vocational) and support families/friends with their loved one's recovery through education individually and through family support group. Able to conduct case management review and aftercare supports. Provide resource information and presentations to professional and community groups on topics related to EPI and mental health.
- Participate on community boards and committees related to mental health and addiction issues.
- Establish and maintain positive relationships with referral sources (e.g. physicians, nursing staff, and colleagues). Provide appropriate feedback to them regarding progress with clients through the course of those involvements.
- Utilization of written reports on consultations, assessments, and the provision of progress notes and termination summaries for client's files.
Mandatory Qualifications
- Master’s degree in social work from an accredited university.
- Member of good standing with the Ontario College of Social Workers and Social Service Workers.
- Completion and regular updates of the Non-Violent Crisis Intervention program.
- Minimum of 5 years of recent experience in mental health services.
- Preference will be given to hospital, outpatient/community and substance abuse treatment experience.
Mandatory Competencies
- Excellent psychosocial assessment
- Treatment skills of client of the service/hospital utilizing modalities of intensive case management, individual, marital/family and group therapies (specifically Cognitive Behavior Therapy (CBT) and CBT for psychosis), and the understanding of community-based resources are essential
- Excellent case coordination skills and ability to consult and provide peer consultation on caseload with peers
- Analytical and organizational skills and demonstrated ability to handle and respect confidential information with tact and diplomacy are essential.
- Ability to work effectively with hospital colleagues and community partners
- Valid driver's license and reliable vehicle.
