Job description

Social Worker, Diabetes Education Centre

As a Social Worker, you will provide psychosocial assessments, counseling, case management, and therapeutic interventions for pediatric, type 1, and gestational diabetes patients and their families. You will develop goal-oriented care plans, act as a resource for diabetes cases, and collaborate with the inter-professional team to support patients with diabetes and co-morbidities.

Key responsibilities include:

  1. Connecting patients with community resources and support networks.
  2. Participating in program planning, resource development, best practice initiatives, and hospital committees.
  3. Leading patient care initiatives and advancing diabetes education.
  4. Mentoring and teaching Social Work interns and students.
