The Position:

We are seeking a Social Worker to support our Day Program and Inpatient Rehab and Complex Continuing Care (CCC) services in delivering exceptional rehabilitation services to children and youth.

The Day Program is an intensive outpatient rehabilitation service designed to improve the quality of life for children and families with diverse healthcare needs. It offers a structured, comprehensive approach focusing on physical, cognitive, and emotional well-being. Clients are medically stable but require ongoing care, allowing them to attend school, participate in therapy, and return home daily.

Inpatient services include Brain Injury Rehab (BIRT), Specialized Orthopedic and Developmental Rehab (SODR), and CCC. Learn more at our inpatient programs.

This is a temporary part-time position (0.8 FTE or 30 hours/week), projected to continue through September 2026.

Key Responsibilities:

  • Conduct strengths-based biopsychosocial assessments for clients and families from diverse backgrounds.
  • Support clients and families to enhance coping and advocacy skills in collaboration with the interdisciplinary team.
  • Identify needs for social work intervention through screening and case finding.
  • Respect and support client/family rights to make decisions aligned with their values and experiences.
  • Provide counseling utilizing evidence-based techniques tailored to mental health concerns.
  • Coordinate referrals and support system navigation for funding, community resources, and healthcare services to facilitate successful discharge.
  • Advocate for client and family needs within Holland Bloorview and the community.
  • Follow established policies, procedures, evidence, and best practices.
  • Accurately document workload statistics.
  • Participate in program and staff meetings.
  • Contribute to quality improvement, education, research, and maintain a safe work environment.
  • Adhere to documentation policies and communicate effectively and timely.
  • Assist in developing program activities and uphold professional standards and ethics.
  • Identify learning needs, set annual goals, and stay current with literature and best practices.
  • Share knowledge and best practices with colleagues, staff, students, and the community.

Qualifications:

  • MSW Degree from an accredited university, Faculty of Social Work.
  • Current member of the Ontario College of Social Workers and Social Service Workers.
  • Membership in the Ontario Association of Social Workers preferred.
  • Paediatric social work or healthcare experience preferred.
  • Experience with youth with disabilities and their families is an asset.
  • Knowledge of psychotherapy frameworks such as ACT, ACBT, DBT, and Solution-Focused Therapy, with the ability to apply them appropriately.
  • Experience working in an interdisciplinary setting preferred.
  • Ability to work effectively as part of a team.
  • Ability to manage and respond to crises calmly and effectively.
  • Strong organizational skills and ability to meet deadlines.
  • Demonstrates initiative and self-direction.
