Indigenous Interprofessional Primary Care Team (IIPCT) is seeking a Registered Social Worker to provide culturally safe, front-line social work support within a collaborative primary care setting. You will support FNIM individuals and families with social, emotional and mental health needs.
The role requires a Master’s in Social Work or 3+ years of direct experience, and involves travel between IIPCT sites in the Deseronto area. Preference given to Indigenous applicants.

Reporting to the Clinic Manager, the Registered Social Worker collaborates with our Mental Health Team to provide front-line support to individuals and families accessing primary care services at IIPCT. This position combines culturally responsive practice with evidence-based social work to support individuals through a wide range of social, emotional, and mental health needs.
