Creston Health Centre (CHC) is seeking a registered Social Worker to join our Home Health team. This position focuses on providing coordinated, accessible, and client-centered care to individuals requiring medical, personal, and social support in their homes. Work hours are rotating 08:00 to 16:00 and/or 16:00 to 22:00. A valid driver’s license is required, with the possibility of driving a personal car on mileage or being assigned a fleet vehicle.
