Relief (temporary), full-time Registered Social Worker (BSW) position available at the Kootenay Boundary Regional Hospital in Trail, B.C. This role supports the Renal program. Workdays are Monday to Friday, 08:30-16:30.

About The Job

The Social Worker provides comprehensive assessment, care planning, and treatment coordination for designated clients and their families. Working collaboratively with other healthcare providers and the multidisciplinary team, the Social Worker offers direct psychosocial care, counseling, group work, system navigation, advocacy, education, and linkage to relevant specialized complex care resources, while facilitating client self-management. The role involves providing and developing social work services to maintain or improve the health, social well-being, and functioning of the identified population.
