A leading healthcare provider in Canada is seeking a Social Worker to deliver psychosocial care and advocacy for residents. The role involves collaboration with healthcare teams to enhance the health and well-being of patients through direct services and system navigation. Ideal candidates will have a Bachelor's in Social Work and relevant experience in healthcare settings.

The requirement to continuously improve quality and safety is inherent in all aspects of this position.
Reporting to the Clinical Coordinator or Social Work Manager, the Social Worker provides assessments and services to meet the emotional and social needs of an assigned group of residents and their families. Working collaboratively with other healthcare providers and the multidisciplinary team, the Social Worker offers direct psychosocial care, system navigation, and advocacy. The role involves developing and delivering social work services aimed at maintaining or enhancing the health, social well-being, and functioning of residents.
An equivalent combination of training and experience may be considered.
