Job description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health), patient and staff safety is a priority and a shared responsibility. The requirement to continuously improve quality and safety is inherent in all aspects of this position.

Position Overview

Reporting to the Clinical Coordinator or Social Work Manager, the Social Worker provides assessments and services to meet the emotional and social needs of an assigned group of residents and their families. Working collaboratively with other healthcare providers and the multidisciplinary team, the Social Worker offers direct psychosocial care, system navigation, and advocacy. The role involves developing and delivering social work services aimed at maintaining or enhancing the health, social well-being, and functioning of residents.

Qualifications: Education, Training, and Experience
  • Bachelor's Degree in Social Work from an accredited university
  • Registered and in good standing with the British Columbia College of Social Workers
  • At least two years of experience in a healthcare setting, preferably in Gerontology or Chronic Diseases
  • Working knowledge of healthcare-related legislation, community resources, and programs

An equivalent combination of training and experience may be considered.

  • Assessment and Treatment: Ability to conduct initial and ongoing client assessments using clinical and diagnostic reasoning, with appropriate interventions.
  • Teaching: Ability to educate clients and others both individually and in groups.
  • Knowledge Integration: Incorporate current research and best practices into professional decision-making.
  • Communication: Effectively communicate with clients, families, medical staff, and the interdisciplinary team through verbal, written, and electronic means. Ability to apply conflict resolution skills.
  • Critical Thinking: Analyze data from multiple sources to problem-solve and make effective decisions, using a systems approach.
  • Human Caring and Relationship-Centered Practice: Promote client-focused care, demonstrating sensitivity to diverse cultures, advocacy, and social justice.
  • Management: Organize work, set objectives, prioritize tasks, and foster cooperation within the team.
  • Leadership: Lead change initiatives, implement policies, and pursue ongoing professional development.
  • Teamwork: Foster collaboration and a commitment to excellence in client care.
  • Equipment Skills: Proficient in computer use, including Windows-based programs, email, and word processing, with the ability to operate related equipment.
  • Physical ability to perform the duties of the position.
