Job summary

Vancouver Coastal Health (VCH) is seeking a Social Worker to address client and family-centred care needs within an integrated health service team. You will work in accordance with VCH policies and a trauma-informed, recovery-oriented model of care across sites, clinics and services.

The role includes comprehensive assessments, care coordination, and collaboration with diverse health professionals to support transitions through the continuum of services for complex discharge and intake needs.

Qualifications

• Master’s Degree of Social Work from an accredited school.
• Current full registration with the British Columbia College of Social Workers.
• Two (2) years experience in healthcare with complex mental health, substance use or chronic medical concerns.

Responsibilities

• Conducts comprehensive social and psycho-social assessments.
• Collaborates with clients and interdisciplinary team to facilitate transitions of care.
• Prepares and maintains client records in accordance with standards and procedures.
• Provides consultative and direct services on issues such as guardianship, directives, housing, or government agencies.
