Job Summary:

Reporting to the Manager of the Mental Health and Addictions Program (MHAP), the Social Worker is responsible for coordinating, developing, implementing, and evaluating case management services associated with the RAAM Clinic. Key responsibilities include:

  1. facilitating staged screening and assessment;
  2. providing case management and system navigation services for the program clients as needed;
  3. creating/planning/facilitating group education;
  4. creating treatment plans to support clinic patients through all stages of the substance use journey.

The RAAM Social Worker will be a leader in addictions advocacy and a strong proponent of harm reduction. The RAAM Social Worker is passionate about helping to facilitate positive change and aligned with the values of SLMHC and RAAM: compassion, teamwork, respect, quality, inclusivity, accessibility, self-determination, and dignity.

Qualifications:

  • Post-secondary education in Social Service Work, Social Work, or equivalent.
  • Registration with Ontario College of Social Workers and Social Service Workers, or eligible to obtain.
  • Experience with Ontario staged screening and assessment tools (certified or willing to become certified to administer GAINS Q3).
  • Experience supporting patients with substance use concerns including the ability to engage with empathy and compassion is an asset.
  • Knowledge and understanding of local community challenges and resources related to substance use is an asset.
  • Knowledge and understanding for First Nations culture and communities is an asset.
  • Ability to analyze and interpret the needs of clients and offer the appropriate options, solutions, and resolutions.
  • Excellent skills in all MS Office.
  • Experience in healthcare organization is an asset.
  • Exceptional conflict resolution and interpersonal skills.
  • Able to respond quickly in a dynamic and changing environment.
  • Highly flexible, with excellent interpersonal skills that allow one to work effectively within an interdisciplinary team, and with patients, families, and communities.
  • Training or experience in SMART Recovery, solution-focused brief intervention, relapse prevention, treatment planning, crisis or suicide intervention, and/or motivational interviewing is an asset.
  • Able to build and maintain lasting relationships with other departments, communities, fellow employees, and patients.

Job Duties:

  1. Work with the multidisciplinary RAAM team to identify patient needs and create individualized treatment plans with patients and support persons.
  2. Provide solutions-focused and trauma-informed, brief intervention and case management services to RAAM clients.
  3. Provide RAAM clients with therapeutic support, nonjudgmental care, rapid access, and systems-navigation.
  4. Create, plan, organize, and facilitate substance use-related group programming and community promotion/advocacy.
  5. Maintain an updated knowledge of services provided by community resource agencies and work cooperatively and collaboratively with other members in the circle of care.
  6. Participate in program development and quality improvement initiatives.
  7. Maintain accurate patient care documentation and statistics in accordance with hospital and professional standards.
  8. Act in accordance with the process and procedures of SLMHC and the ethical standards of OCCSWSW.
  9. Provide crisis intervention and/or referral to appropriate counselling or therapy services.
  10. Maintain communication with patients and the interdisciplinary healthcare team.
  11. Advocate for substance use patients and services which help to advance harm reduction and patient-centered, evidence-based addictions care.

Salary: The salary range for this position will be in line with the OHA compensation structure.
