As a Social Worker (Temporary, Full-Time, effective Immediately to February 12, 2027; Location Sudbury) you are responsible for the provision of social work services to patients, in order to achieve optimum psychosocial functioning, prevent unnecessary deterioration, and promote patient independence.

Whether you work in our office or in the community, or a health care facility - you will play a lead role in providing connected, accessible, patient-centered care - and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.

What will you do?
  • Respond to referrals from Care Coordinators to appropriately administer social work services to patients.
  • Assess patients to identify and determine psychosocial needs and therapeutic treatment needs and goals.
  • Provide treatment program, counseling strategies, and emotional/mental health support to patients.
  • Oversee patient’s progress and adjust treatment programs when needed, to ensure therapy goals are met.
  • Educate and counsel patients, their families, caregivers, and/or SDMs regarding issues, challenges and strategies for continuous improvement.
  • Accurately document and chart patient’s assessment results, goals, treatment plans, and interventions.
  • Link patients with other in-house services or outside organizations, if needed.
  • Balance patient needs and choices with available resources, ensuring their values and preferences are respected.
What must you have?
  • Registered Social Worker in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Relevant social work experience in a community/health care environment.
  • Comprehensive knowledge of social work interventions, practice methods, and professional skills.
  • Knowledge of other community resources, funding agencies, treatment centers, social service networks, and organizations.
  • Good understanding of the roles of other health care professionals.
  • Up-to-date knowledge of social work theories, practices, and procedures.
  • Ability to prioritize professional duties and manage caseload in a time efficient manner.
  • Strong interpersonal skills.
  • Effective listening, observation, and facilitation skills.
  • Strong mediation, negotiation, and conflict resolution skills to respond or assist in emergency situations.
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Outlook).
  • Must have valid driver’s license and access to a reliable vehicle.
  • Advanced oral and written proficiency in English and French is essential.

Bilingual candidates with advanced oral and written proficiency in English and French will be given first consideration; however, less qualified candidates who don’t meet the language requirements may still be considered.

What would give you the edge?
  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
