Description
The Social Worker will provide information, assessment, and entry facilitation into Reena’s Community Participation Supports and Residential Systems. They will act as the contact for individuals who have chosen Reena as their lead agency. The role involves participating in decision-making processes for individuals within Reena’s system and supporting intervention in cases of complex needs.
This role will contribute to internal and external committees to coordinate services within the Developmental Services Ontario (DSO) scope, the Ministry of Health and Long-term Care, and Ontario Health Teams.
Key Responsibilities :
Intake Support Responsibilities
- Provide information to individuals seeking or referred to Reena for services via email and phone, redirecting to the Intake Social Worker as needed.
- Support intake processes, including preparing necessary documentation.
- Conduct meetings with families and agencies to plan available options.
- Support Intake Social Workers as needed.
- Collaborate with stakeholders and departments for smooth transitions.
- Handle internal transfers of supported individuals.
- Assist in tracking applications for Passport Funding.
- Respond to inquiries about Reena’s residential, community supports, and DSO services.
- Manage internal agency requests like DSO assessments on DSCIS.
- Respond to Ontario Health Teams inquiries for warm transfer services.
- Attend planning meetings and assist with new build applications.
- Perform other duties as assigned.
System Responsibilities
- Stay informed about system changes and sector trends.
- Familiarity with Access Point and similar services.
- Represent the agency at sector-wide, municipal, or provincial meetings alongside leadership.
- Instruct courses for Learning and Development.
- Advise on course development based on sector developments and research.
- Act as a resource for staff and management.
- Follow relevant policies and legislation, including those from the Ministry of Children, Community and Social Services, and the Ministry of Health.
- Manage passport program applications.
- Perform other duties as assigned.
Self-Development
- Seek new information and opportunities to enhance skills.
- Take actions to improve abilities aligned with career goals.
- Maintain currency with the Ontario College of Social Worker and Social Service Workers (OCSWSSW) requirements.
Planning / Organizing and Flexibility
- Manage multiple tasks across teams and programs.
- Organize resources for internal groups and committees.
- Notify and collaborate with relevant parties about potential issues.
- Show flexibility in routines and scheduling based on priorities.
Qualifications :
- Bachelor of Social Work (mandatory).
- Annual renewal & registration with OCSWSSW, in good standing.
- At least 2 years’ experience in health and developmental services sectors.
- Knowledge of the intake process.
- Knowledge of the mental health sector is an asset.
- Experience working directly with families.
- Verifiable knowledge of the community served.
- Experience in needs assessment to identify service gaps.
