Job summary

Summerland Seniors Village in Summerland, BC, seeks a Part-Time Social Worker to deliver social work services to residents and families across long-term care programs. You will assist with admissions, discharges, and care conferences.

Reporting to the General Manager, you will support families, coordinate resources, and participate in quality improvement initiatives. Experience in long-term care and BC registration are required.

Qualifications

  • Bachelor's degree in Social Work is required.
  • Master's in Social Work is an asset.
  • 3 years of recent related experience in long-term care is preferred.
  • Current registration with the BC College of Social Workers is required.
  • Must be able to think analytically and identify causal links to events.
  • Must be flexible and adapt in various situations.
  • Must be an active listener and understand residents' needs.
  • Computer skills include Word, Excel, Outlook; familiarity with basic programs.

Responsibilities

  • Deliver social work services to residents and families in LTAC and convalescent care.
  • Assist with admissions to and discharges from the facility.
  • Participate in resident care conferences and provide ongoing support.
  • Contribute to continuous quality improvement of services.
  • Coordinate with health authorities and local physicians when needed.
  • Assist with discharge planning and community resource referrals.

Position: Part-Time Temporary - 16 hours per week (5 month term)

About the Role

Reporting to the General Manager and/or designate, the Social Worker is responsible for thedelivery of social work services to residents and their families in all Long Term Care andconvalescent care programs. Informs resident and family of available services and of residentrights and responsibilities. Performs key role in admissions to and discharges from the facility.Participates in resident care conferences and provides ongoing social work support to familiesand residents. Participates in continuous quality improvement of the services provided.

Responsibilities
  • 1. Be an engaged collaborative team player, developing and maintaining professionalrelationships that support and strengthen the organizational culture and purpose.
  • 2. Continually evaluates and utilizes best practice information/research to improve thequality of assigned services.
  • 3. Participates in determining suitability of applicants and selecting appropriate residentsfor available vacancies.
  • 4. Assesses and assists residents and families where there is an indication of a psychosocial problem, involving emotional, behavioural and or cognitive concerns.
  • 5. Completes incapability assessments.
  • 6. Leads referrals to Public Guardian and Trustee, and support residents/families with financial matters.
  • 7. Assists with coordination of admission of new residents to the community.
  • 8. Prepares resident social/psychosocial histories for initial care conferences.
  • 9. Provides consultation and education to residents and their families and directs themtoward appropriate government and community resources.
  • 10. Supports educational in-service training for staff related to resident rights and socialservices.
  • 11. Liaises with the Health Authority and other local community physicians.
  • 12. Assists in discharge planning for convalescent and complex care clients.
  • 13. Assists with arrangements for resident out-of-community health care, transportation andcounselling services.
  • 14. Participates in the community’s Continuous Quality Improvement Program andAccreditation processes.
  • 15. Attends resident care conferences as required, team rounds, and family meetings andpresents information related to the social welfare of the residents.
  • 16. Maintains and updates resident records as appropriate and in accordance withcommunity policies.
  • 17. Facilitates Family Council in conjunction with the leadership staff.
  • 18. Participates in critical incident debriefing as deemed appropriate by the leadership staff.
  • 19. Participates in planning and monitoring program development including palliative care.
  • 20. Participate in a variety of committee meetings as assigned through terms of reference(e.g. LTC Best Practice).
  • 21. Fulfils leadership role in implementing and maintaining ethics protocols in collaborationwith the leadership team.
  • 22. Actively participates in orientation of new staff, community programs, meetings,committees and educational in-services.
  • 23. Maintains a safe environment for residents, family and staff, through: - accident prevention, - knowledge of emergency procedures and regulations, - the safe use of supplies and equipment and reporting when repairs arenecessary, and - Utilizing infection control procedures
  • 24. Performs other related duties as required.
Qualifications & Skills
  • 1. Bachelors degree in Social Work is required.
  • 2. Masters in Social Work an asset.
  • 3. 3 years of recent related experience - long term care setting preferred.
  • 4. Current registration with the BC College of Social Workers.
  • 5. Ability to think analytically and recognize likely causal links to events while anticipateobstacles and solve problems effectively.
  • 6. Ability to be flexible and adapt in a variety of situations and groups and can managechange effectively.
  • 7. Ability to be an active listener and understand the needs of all persons served.
  • 8. Computer skills including the ability to navigate basic computer programs such as Word,Excel, Outlook.
  • 9. All successful applicants must pass the vulnerable sector Criminal Record Checkapplicable to Provincial guidelines.
  • 10. Ability to read, write, and speak English proficiently and understand verbal and writteninstructions.
