Position: Social Worker - Part Time
Key Responsibilities
- Receive and respond appropriately to inquiries regarding new resident placement, including conducting tours of the home with prospective residents.
- Conduct pre-admission procedures such as screening applications, corresponding with applicants, conducting pre-admission visits and assessments, communicating with placement agencies, and guiding residents and families through the admission process in coordination with the LHIN.
- Promote a resident-oriented environment, acting as a resident and family advocate. Coordinate and serve as the liaison for the Family Council. Lead the home's Customer Service program.
- Coordinate, plan, and participate in interdisciplinary team care conferences for residents, including new admissions.
- Plan and implement social work programs to meet the psychosocial needs of residents and their families or advocates.
- Provide palliative care support to residents and families as needed.
- Address issues of financial or emotional abuse collaboratively with the interdisciplinary team.
- Lead the home's ethics committee and provide consultation to staff on ethical dilemmas, working collaboratively to resolve issues.
- Participate in committees related to resident care and facility operations as required.
- Work as part of the multidisciplinary team in planning overall resident care.
- Provide educational and consultative services on social work topics through staff orientation, development programs, and seminars for staff, residents, and families.
Key Qualifications
- Registered under the Social Work & Social Service Act, 1998; Bachelor of Social Work degree from a recognized school.
- Experience working with geriatrics; familiarity with multidisciplinary team concepts; effective teamwork skills.
- Minimum of 3 years' experience as a social worker in a clinical setting.
- Strong knowledge of mental health issues.
- Excellent interpersonal and communication skills; team player.
- Ability to respond effectively to crisis situations with critical thinking.
- French language skills are considered an asset.
