Home: West Park

Location: St. Catharines, ON

Position: Social Worker - Part Time

Key Responsibilities
  1. Receive and respond appropriately to inquiries regarding new resident placement, including conducting tours of the home with prospective residents.
  2. Conduct pre-admission procedures such as screening applications, corresponding with applicants, conducting pre-admission visits and assessments, communicating with placement agencies, and guiding residents and families through the admission process in coordination with the LHIN.
  3. Promote a resident-oriented environment, acting as a resident and family advocate. Coordinate and serve as the liaison for the Family Council. Lead the home's Customer Service program.
  4. Coordinate, plan, and participate in interdisciplinary team care conferences for residents, including new admissions.
  5. Plan and implement social work programs to meet the psychosocial needs of residents and their families or advocates.
  6. Provide palliative care support to residents and families as needed.
  7. Address issues of financial or emotional abuse collaboratively with the interdisciplinary team.
  8. Lead the home's ethics committee and provide consultation to staff on ethical dilemmas, working collaboratively to resolve issues.
  9. Participate in committees related to resident care and facility operations as required.
  10. Work as part of the multidisciplinary team in planning overall resident care.
  11. Provide educational and consultative services on social work topics through staff orientation, development programs, and seminars for staff, residents, and families.
Key Qualifications
  • Registered under the Social Work & Social Service Act, 1998; Bachelor of Social Work degree from a recognized school.
  • Experience working with geriatrics; familiarity with multidisciplinary team concepts; effective teamwork skills.
  • Minimum of 3 years' experience as a social worker in a clinical setting.
  • Strong knowledge of mental health issues.
  • Excellent interpersonal and communication skills; team player.
  • Ability to respond effectively to crisis situations with critical thinking.
  • French language skills are considered an asset.
