Job summary

Regent Park Medical Clinic in Regina, SK is seeking a Temporary Full-Time Social Worker to support patients' psychosocial needs, provide counseling, advocacy and resources navigation within a team-based primary care setting.

Reporting to the Lead Physician, you will develop treatment plans and conduct assessments, plus offer individual, group and family counseling while collaborating with other health professionals to improve access and quality of care.

Qualifications

• Master's degree in social work or equivalent required.
• 1 year of experience in homecare, community health, case management or primary health care setting.
• Membership in good standing with SASW.
• Ability to work independently and in a multidisciplinary team.

Responsibilities

• Identify, implement and evaluate required services to support individuals at risk.
• Receive and review referrals and consult with staff to identify clients who need Social Work.
• Support clients and families to manage stress from illness, injury or disability.
• Develop individualized treatment plans.
• Provide individual, group and family counselling sessions.
• Offer crisis intervention and support during emergencies.
• Maintain detailed and accurate client records.

Social Worker - Regina SK

Regent Park Medical Clinic is seeking a Temporary Full-Time Social Worker. Reporting to the Lead Physician, the Social Worker will be responsible for supporting patients with their psychosocial needs, as well as providing counseling, advocacy and resources navigation to individuals and families, contributing to holistic patient care.

If you have a university degree or masters in social work and at least 1 year of experience in homecare, community health, case management or a primary health care setting, then this job is for you!

This position is part of a newly developed Innovation Fund between the Saskatchewan Government and medical clinics across Saskatchewan with the aim of improving patient access and quality care through team-based support, that will deliver better access to primary care in the province of Saskatchewan. This position is temporary until March 31, 2028, with the possibility of an extension.

Responsibilities

• Identify, implement and evaluate required services to support individuals identified as at risk
• Receives and reviews referral information and consults with other staff/team to identify clients/families who would benefit from Social Work services and are appropriate for Social Work involvement.
• Support clients/families to manage the stresses that arise because of illness, injury or disability
• Develop individualized treatment plans
• Provide individual, group and family counselling sessions
• Offer crisis intervention and support during emergencies
• Maintain detailed and accurate client records
• Advocate for clients needs, educate and connect them with appropriate resources
• Collaborate with other healthcare professionals within the clinic
• Promote cultural competence and sensitivity in service delivery
• Participate in clinic quality improvement projects as applicable
• Participate in case management and follow-up services
• Conduct assessment to identify the needs of families during the perinatal period
• Conduct depression screening with prenatal and postpartum patients during intake and follow-up (postpartum)
• Attend and participate in clinic huddles

Qualifications

• Masters degree in social work
• Will consider Bachelor of Social Work (BSW) candidates with relevant experience
• Membership in good standing with the Saskatchewan Association of Social Workers (SASW)
• Minium of 1 year of experience in healthcare, homecare and community setting
• Demonstrated skill and experience in clinical counseling
• Excellent knowledge of pertinent community resources, support and services typically used by health care clients
• Familiarity with an evidenced-based practice and continuous quality improvement environment
• Knowledge of ages/stages of life cycles, mental illness/ mental health
• Ability to provide standard Social Work service including assessments and consultation, case management, psychosocial counseling, crisis intervention, referral and discharge planning when applicable
• Experience working with diverse populations
• Ability to work with clients/families utilizing a strength-based client focused approach to care and service delivery
• Strong Oral and written communication skills
• Ability to work independently and the ability to function as part of a multidisciplinary team
• A valid Saskatchewan Drivers License
• Local to the area and knowledge of local community resources and services is considered as asset
• Demonstrated ability to work collaboratively within in a multidisciplinary setting
• Demonstrated problem solving and decision‑making skills
