A leading healthcare provider is seeking a compassionate Social Worker to support residents and families at their Continuing Care home in Edmonton. In this part-time role, you will coordinate care and advocate for patients in need, ensuring they receive the support necessary for their well-being. Hours of Work: Mondays, Tuesdays, Wednesdays, Fridays 8:00am - 4:15pm
31 hours per week, 0.80 FTE

Reporting to the Director, Resident Care, Tatsiana Haidukevich, the Social Worker will provide support to Residents and Families by coordinating legal processes, liaising with community resources, coordinating companion care, advocating on Resident/Family behalf with Physicians, Resident care team members, and external agencies; providing counselling and/or information on disease progression, goals of care, and end-of-life care; and contributing to the development and review of Resident-specific plans of care.
DUTIES AND RESPONSIBILITIES:
The Social Worker also assists residents and families with applications for financial support to applicable senior support programs.
Our Ideal Candidate Qualifications:
Preference will be given to applicants with previous Social Work experience in a health/long-term care setting, caring for seniors, and assisting people living with dementia and complex care needs.
