Reporting to the Director, Resident Care, Tatsiana Haidukevich, the Social Worker will provide support to Residents and Families by coordinating legal processes, liaising with community resources, coordinating companion care, advocating on Resident/Family behalf with Physicians, Resident care team members, and external agencies; providing counselling and/or information on disease progression, goals of care, and end-of-life care; and contributing to the development and review of Resident-specific plans of care.

DUTIES AND RESPONSIBILITIES:

  • Provide support to Residents and Families by:
  • Coordinating legal processes (such as enactment of Personal Directives, Enduring Power of Attorney, and appointment of Guardianship & Trusteeship);
  • Liaising with community resources;
  • Advocating on Resident/Family behalf with Physicians, Resident care team members, and external agencies;
  • Providing counselling and information on disease progression, goals of care, and end-of-life care;
  • Contributing to the development and review of Resident-specific plans of care;
  • Providing education to families on dementia and supporting the care team in responsive behavior management.

The Social Worker also assists residents and families with applications for financial support to applicable senior support programs.

Our Ideal Candidate Qualifications:

  • Diploma or degree in Social Work from a recognized post-secondary educational institution.
  • Current member in good standing with the Alberta Association of Registered Social Workers.
  • Excellent communication, interpersonal, problem-solving, and leadership skills.
  • Ability to work effectively with all team members.
  • Passion for working with older adults and enhancing their quality of life.

Preference will be given to applicants with previous Social Work experience in a health/long-term care setting, caring for seniors, and assisting people living with dementia and complex care needs.

All successful candidates must have, prior to start date:

  • A current and clear Criminal Records Check with Vulnerable Sector Check obtained within 90 days prior to hire.
  • A tuberculosis screening completed within 12 months prior to hire.
