St. Michael's Health Group is seeking a compassionate Social Worker to join their team. The ideal candidate will possess a Social Work degree, have strong interpersonal skills, and a passion for enhancing the lives of older adults. The role involves providing support to residents and families, coordinating care, and promoting diversity and inclusion within the community.
Hours of Work : Mondays, Tuesdays, Wednesdays, Fridays 8 : 00am - 4 : 15pm
31 hours per week, 0.80 FTE

Reporting to the Director, Resident Care , Tatsiana Haidukevich, the Social Worker will provide support to Residents and Families by coordinating legal processes, liaising with community resources, coordinating companion care, advocating on Resident / Family behalf with Physicians, Resident care team members, and external agencies; providing counselling and / or information on disease progression, goals of care, and end-of-life care; and contributing to the development and review of Resident-specific plans of care.
DUTIES AND RESPONSIBILITIES :
The Social Worker also assists residents and families with applications for financial support to applicable senior support programs.
Our Ideal Candidate Qualifications :
Preference will be given to applicants with previous Social Work experience in a health / long-term care setting, caring for seniors, and assisting people living with dementia and complex care needs.
