Job summary

Qualifications

• Bachelor of Social Work from an accredited university (clinical degree) required
• Masters of Social Work from an accredited university (clinical degree) preferred
• Registration with the Ontario College of Social Workers and Social Service Workers required
• Membership with the Ontario Association of Social Workers preferred
• 2 years of acute care hospital social work experience preferred
• Excellent interpersonal and communication skills
• Ability to work under pressure, prioritize demands and meet deadlines
• Proven flexibility and ability to adapt to changing patient needs
• Demonstrated computer skills

Responsibilities

• Maintains current working knowledge of relevant legislation (e.g., Mental Health Act, Health Care Consent Act, Substitute Decisions Act, PHIPA, Excellent Care for All Act)
• Maintains expertise in generic social work practice and evidence-based approaches
• Documents all patient interventions per standards and guidelines
• Conducts comprehensive psychosocial assessments from a systems perspective
• Identifies and intervenes with high-risk social situations (e.g., IPV, elder/financial abuse)
• Provides theory-based counselling, crisis intervention and illness adjustment counselling
• Coordinates complex discharges and facilitates program-specific case management
• Leads discharge planning and mentors staff on areas of expertise
• Supports patient and family education and connects with community resources
