Job summary

Renfrew Victoria Hospital in Ontario is seeking a permanent, full-time Social Worker to support patients with chronic kidney disease within the Nephrology Program. The role provides comprehensive, quality patient and family centered social work services aligned with the Ontario Renal Plan and RVH departmental goals.

Duties include psychosocial assessment, care planning with the multidisciplinary team, assisting with medication coverage and financial resources, and ensuring goals of care

Qualifications

  • Current Certification with the College of Social Workers and Social Service Workers of Ontario.
  • BSW or MSW degree from a recognized program.
  • Experience with chronic diseases is an asset.
  • Excellent communication and interpersonal skills.
  • Ability to work in a team and independently.

Responsibilities

  • Provide social work services to people at all stages of Chronic Kidney Disease within the Nephrology Program.
  • Assess psychosocial needs and coordinate care with the multidisciplinary team.
  • Assist patients with medication coverage and other financial assistance resources.
  • Support goals of care discussions and proper documentation.
  • Promote timely access to relevant community resources (e.g., Kidney Foundation, ODSP, etc.).

Come join us! We are hiring a permanent, Full-Time Social Worker with our Nephrology Program.Pay Grid: $43.89 - $53.34 /hour

Working as a member of the clinical team and under the direction of the Clinical Manager, the incumbent provides and supports patients with chronic kidney disease. The Social Worker provides comprehensive, quality patient/family centered social worker services that are aligned with the Ontario Renal Plan and RVH Nephrology departmental goals.

Duties will include but are not limited to:
  • The Social Worker provides services to people at all stages of Chronic Kidney Disease, including Multicare Kidney Clinic, Peritoneal Dialysis, Hemodialysis, and Conservative Kidney Care. The Social Worker has the knowledge, skills and judgment needed to provide competent, evidence-based Social Work practice
  • The Social Worker shall be responsible for meeting the standards and exercising judgment for providing safe, competent and ethical Social Work care as outlined in the Code of Ethics as set out by the Ontario
  • College of Social Workers and Social Service Workers and the Renfrew Victoria Hospital Policies, Procedures and Protocols, and the standards of care appropriate to his/her area of clinical practice.
  • To assess the psychosocial needs of Nephrology patients and their families affected by renal disease and its treatment and bring this perspective to the multi-disciplinary team.
  • To provide appropriate supportive care pertaining to the emotional/social consequences of medical treatment, (such as dialysis and kidney transplantation), hospitalization, illness and/or issues related to current or historical stressors.
  • To intervene on a preventative basis with patients recently diagnosed with renal disease to promote changes in lifestyle and coping mechanisms to enable them to achieve optimal health and independence. · to promote timely access to relevant community resources such as Kidney Foundation, Transportation services, Ontario Health at Home, ODSP, etc.
  • To assist patient who are being worked up for transplant to get to necessary appointments
  • To work as a liaison between the multi-disciplinary team and the patient or family.
  • To support the Nephrology team to ensure that all patients have goals of care discussions and documentation, and to submit data to Ontario Renal Network.
  • Assist patients with application for medication coverage and other financial assistance resources.
Qualifications:
  • Current Certification and in good standing with College of Social Workers and Social Service Workers of Ontario.
  • Knowledge acquired through the successful completion and graduation of a recognized BSW or MSW program.
  • Experience working with people with chronic diseases is an asset.
  • Excellent communication and interpersonal skills.
  • Ability to understand, interpret and carry out written and verbal instruction.
  • Ability to set priorities.
  • Ability to work effectively and provide care within a team environment and contribute to and work with a patient centered plan of care.
  • Ability to work independently and to perform duties within the physical demands of the work area.
  • Basic knowledge of established Infection Control and Occupational Health & Safety practices.
  • Bilingualism will be considered an asset.
